Take the White Bear Avenue exit off of Hwy 36, then turn North onto White Bear Avenue.

Please bring your child’s health insurance card/s to your first visit. It is also helpful to bring any recent reports from other providers that may pertain to occupational therapy or related services, or to your child’s diagnosis or condition.
